About LTSS
The LTSS Assessment is a uniform assessment used for all MassHealth members enrolled in or seeking eligibility for the Community Case Management (CCM) program. The LTSS Assessment determines a member’s need for CSN services by reviewing and quantifying the time it takes to complete all medically necessary nursing interventions.
Eligibility
Age: 3-64
Insurance: MassHealth + ACO or MCO
Other Qualifiers: Complex LTSS or BH needs
Actively lives in CAWM service area.
How to enroll
Enrollment for BHCP in completed via the smart sheet.
How to document
Smart Sheet
- On the status column select Pending
- on the comments/information column 12/05/2024 ERS Jane Doe referred client to LTSS.
- on the care management column select LTSS
- The LTSS staff will update the smart sheet with information regarding the Eligibility and enrollment. Update the status as the information is provided. in the comments/information column
Carelogic
- 12/05/2024 ERS Jane Doe referred client to LTSS.
- also include all status updates when documenting.
Inform referent
Update the referent via email or phone. Let them know the client has been or will be enrolled into LTSS
